Alluvion Health announced on Monday, March 25, 2024, that its Project in Assistance in Transition from Homelessness (PATH) program is being rejuvenated with a new team of professionals. The program is aimed at providing support to individuals experiencing homelessness, especially those dealing with mental illnesses or substance use disorders. Services offered through the PATH program include outreach, engagement, case management, and service coordination. The goal is to identify eligible individuals, assess their needs, and connect them with resources to address their homelessness.
